Aged care physiotherapy at home

Helping older people across the greater Sunshine Coast stay strong, steady and independent — at home, where it counts.

What the Support at Home program is

Support at Home is the Australian Government's in-home aged care program — it replaced Home Care Packages in November 2025. If you're approved, you receive a quarterly budget, managed through a registered provider, to pay for services that help you stay safe and independent at home.

Physiotherapy sits in the program's clinical supports category. That matters for two reasons: it recognises therapy as health care, delivered by university-qualified professionals — and under current rules there is no participant co-contribution for clinical supports. The cost comes from your budget, not your pocket. Travel is included in our pricing, so there are no extra charges for us coming to you.

Not yet in the program? Access starts with an assessment through My Aged Care — call 1800 200 422 or ask your GP. And if you're waiting on approval, private visits are available in the meantime.

How physiotherapy helps under Support at Home

The program's whole purpose is keeping you at home, living as independently as possible — and that is exactly what physiotherapy works on. Strength, balance, walking and confidence aren't fixed: they respond to the right program, at the right dose, progressed at the right pace.

Common reasons we're brought in: someone has had a fall or a near miss; walking has become slower or less steady; getting out of a chair, bed or car is getting harder; a hospital stay has taken strength away; pain is shrinking someone's world; or a family has noticed their parent doing less and less. None of these have to be accepted as "just ageing" without at least an assessment.

How the funding works

Through your Support at Home provider. Your registered provider or care partner arranges physiotherapy under your service agreement with them, and the cost is drawn from your quarterly budget. Because physiotherapy is a clinical support, no co-contribution applies under current program settings — and our travel is included in the price. The practical step is simple: ask your provider or care partner about adding physiotherapy — or have them contact us directly and we'll sort it out between us.

Privately. No budget yet, waiting on assessment, or want more sessions than your budget allows? Private visits are available at any time — see Fees & funding.

Each of these is explained in more detail on our What We Help With page.

What a home visit looks like

Your first visit. We start with a conversation, not a treatment table: your health history, your medications, what's changed recently, what you're worried about, and what you want to get back to. Then a physical assessment: how you stand, walk, balance, and manage the movements your day depends on, using recognised outcome measures so progress can be tracked properly. You'll finish the visit knowing what we found and what we recommend. Allow 1 to 1.5 hours — we don't rush first visits, especially when things are complex.

Ongoing sessions. A typical session combines targeted exercise with practical work on the tasks that matter — stairs, transfers, walking outdoors — plus hands-on treatment where it's appropriate. You'll have a written home program, kept deliberately realistic: a program only works if it actually gets done. Support workers and family are welcome in every session, and we'd rather they were there — progress is faster when the people around you know the plan.

Reviews and reporting. We re-measure at regular intervals using the same outcome measures from your first visit, so improvement — or early signs of decline — is picked up with numbers, not impressions. A written progress report goes to your provider every 12 weeks, with updates to your GP or family exactly as agreed with you.

For Support at Home providers and care partners

We take brokered physiotherapy referrals from home care providers across the greater Sunshine Coast — including for participants on restorative and therapy-focused pathways.

What you can rely on: referrals acknowledged within 1 business day; assessment summaries after the first visit; written progress reports every 12 weeks, or to your timelines; outcome-measure data you can use in care plan reviews; early, documented escalation of clinical concerns; and current insurance, registration and screening documentation on file for your compliance records. Travel is included in our pricing — no separate travel charges.

We already deliver services alongside several established providers. Contact us to set up an arrangement — we can work within your subcontracting agreements and reporting systems.
