Aged Care (Support At Home)
Physiotherapy at home for elderly people — falls prevention, strength, mobility and recovery after hospital or illness. Under the Support at Home program, physiotherapy is a clinical support, which means it can be funded from your budget with no co-contribution. Private visits are also available.
NDIS
Goal-based physiotherapy for self-managed and plan-managed NDIS participants aged 18–64, delivered at home and out in the community — with the reports and evidence you need at plan review time.
How it works
1. Refer or enquire
Families, support coordinators, care partners and providers can all refer — or simply get in touch yourself. No GP referral is needed, and a phone call is enough to start. Referrals are acknowledged within 1 business day.
2. Assessment at home
Your first visit happens in the place you actually live and move. We take the time to understand your history, your worries and your goals — then assess how you're moving now, in your real environment.
3. A plan that's followed through
Ongoing visits, a home program pitched at a level you can actually manage, and clear written updates to your family, provider or coordinator — so everyone supporting you is working from the same plan.
Why home-based physiotherapy
A clinic can't see how you move at home. We can.
The step into your shower, the distance to your letterbox, the chair you struggle out of — these are the things that decide independence, and they can't be assessed from a treatment room. Working at home also removes the biggest barrier to actually attending therapy: getting there. And because sessions happen where your support network is, family members and support workers can join in, learn the program, and keep it going between visits.

Working with providers and coordinators
We partner with Support at Home providers, support coordinators and plan managers across the Sunshine Coast. That partnership is built on the things that make your job easier: referrals acknowledged within 1 business day, session notes and progress reports in the format you need, early escalation of clinical concerns, and compliance documentation kept current and on file.
